Homeless Management Information System

The Homeless Management Information System (HMIS) is a secure, locally administered database used to collect client-level data on the provision of housing and services to individuals and families at risk of or experiencing homelessness. Utilizing an HMIS allows the Continuum of Care to coordinate and track services between agencies, report on program performance, identify gaps and trends, and make data driven decisions. As the lead agency of the Continuum of Care, Doorways of NW Florida administers the local HMIS, Service Point. HMIS utilization is a requirement for certain programs receiving federal funds. However, CoCs encourage participation from all local providers regardless of funding source.

HUD reporting and System Metrics

Longitudinal Systems Analysis (LSA)

An annual HMIS-generated report submitted to HUD that shows how individuals and families interact with our regional homelessness response system over time.

These six core metrics help gauge the effectiveness of our homelessness response:

    1. Measure 1: Length of Time Persons Remain Homeless

    2. Measure 2: The Extent to which Persons who Exit Homelessness to Permanent Housing Destinations Return to Homelessness
    3. Measure 3: Number of Homeless Persons
    4. Measure 4: Employment and Income Growth for Homeless Persons in CoC Program-funded Projects
    5. Measure 5: Number of Persons who Become Homeless for the First Time
    6. Measure 6: Homeless Prevention and Housing Placement of Persons defined by category 3 of HUD’s Homeless Definition in CoC Program-funded Projects
  • Measure 7: Successful Placement from Street Outreach and Successful Placement in or Retention of Permanent Housing

2024 System Performance Summary Report

Conducted each January, the PIT Count captures a snapshot of both sheltered and unsheltered individuals experiencing homelessness on a single night. This HUD-mandated count occurs at least every two years and helps measure local needs and system reach.

2025 PIT Summary Report

The HIC is conducted alongside the PIT Count and tracks the number of beds and units dedicated to serving people experiencing homelessness. It includes data from five program types: Emergency Shelter, Transitional Housing, Rapid Re-Housing, Safe Haven, Permanent Supportive Housing.

2025 HIC Summary Report

Organizations receiving HUD CoC grants are required to submit an APR annually. This report is generated through HMIS and reflects program outcomes for the operating year.

Recipients of ESG Program funding must submit a CAPER to HUD annually. Like the APR, this report is produced through HMIS and includes performance data across funded activities.
